Position Description

We Are Hiring Registered Nurse-Case Management - Full-Time, Day - Our Lady of Lourdes Memorial Hospital - Binghamton, NY 8 am - 4:30 PM, M-F, rotating weekends and holidays Why Join Ascension? Reaching beyond the main hospital campus, Lourdes has established a network of primary care providers at 12 convenient sites throughout the region; and the Lourdes at Home and Hospice programs which care for patients in their homes and various nursing facilities. Two mobile mammography vans, a dental van and a medical services van deliver health services out into the region, making health care more easily accessible to surrounding communities. What You Will Do As an Associate with Our Lady of Lourdes Memorial Hospital, you will have the opportunity to coordinate the overall interdisciplinary plan of care for patient, from admission to discharge. Responsibilities: * Creates plan for care across the continuum, integrating patient/family preferences and values. * Monitors patient care through assessments, evaluations and/or patient records. * Advocates for resources and removal of barriers. * Maintains ongoing dialog with supervisor and other health providers to ensure effective implementation of health plan. * Acts as a resource for adequate medical record documentation, appropriateness of services as they relate to diagnoses, and treatment options for post-discharge care. What You Will Need Licenses/Certifications/Registration: * Required Credential(s): * Registered Nurse credentialed from the New York Board of Nursing obtained prior to hire date or job transfer date.
